 Founded/ Unfounded

 

Displays all founded and unfounded incidents in chart form. It can be run by school or district.

 

 

 

Incident per Grade

 

Breaks down all incidents by grade level. This report can be run by school or district.

 

 

Incident Victim Ethnicities

 

Breaks down incident victims by ethnicities. This report can be run by school or district.

 

 

Hot Spot Locator

 

Tells you where each incident has happened within the school or outside the school. This report can be run by school or district.

 

 

 

Task Completion Report

 

Provides a summary of the tasks within incidents and their completion status.

 

 

 

Repeat Offender

 

Provides an excel report of all the offenders which have been involved in 2 or more incidents. This report can be run by school or district.

 

 

 

 

 

Repeat Victim

 

Provides an excel report of all the victims which have been involved in 2 or more incidents. This report can be run by school or district.

 

 

Incident Summary 

 

Summarizes all incident’s information including students names and identifying information. This report can be run by school or district.

 

 

Executive Summary

 

Typically run once a month and taken to the BOE (Board of Education) for approval of an incident or incidents. This report provides an incident summary with no identifying student information.  It can be run by school or district.

 

 

 

Incident Tasks 

Inside an incident you will see a tasks tab. This report will show you all the tasks that were completed on time, were late or not completed at all. This report can be run by school or by district.

 

 

Incident Audit

 

Provides a report including all actions taken on an incident. This report can be run by school or  district.

 

 

Incident Signatures

 

When an incident is reassigned, there is a box that has to be checked in order to assign this over to another person. This report will give you a list of everyone that was assigned to each incident and can be run by school or district.

 

 

 

Investigations

 This report will tell you how many incidents were initiated and completed within the 10 day timeline. It will also give you the number of investigations you have had in the school or district.

 

Safety Report

 

This is an all in one report that will breakdown everything down. This safety report will give you a list of all the incidents in the system by the following.

Superintendent HIB Report

 

Provides all non-identifying information about an incident. Similar to an executive summary report in a more compact structure.

 

 

 

Civil Rights Data Collection

 

The Civil Rights Data Collection is a biennial (i.e., every other school year) survey of public schools required by OCR since 1968. The CRDC collects data on leading civil rights indicators related to access and barriers to educational opportunity at the early childhood through grade 12 levels.

 

 

Code of Conduct Report

 

Code of Conduct report includes a list of the total applied demerits to students within the filtered range.

 

Philadelphia Sexual Conduct Report

 

Customized report for Philadelphia school district which counts incidents involving an act of sexual harassment or sexual violence. This includes a count of all schools and an aggregate.
